In The War Within Patch 11.1.5, Revisited Horrific Visions are repeatable instances designed for solo players or small groups of up to five, with no specific roles required (there’s no strict Tank/Healer requirement). Horrific Visions are officially returning in World of Warcraft Patch 11.1.5, launching on May 20, 2025. This updated version revives one of Battle for Azeroth 8.3 activities. Although Patch 11.1.5 launches on April 22, Horrific Visions Revisited won’t be available until nearly a month later.
Horrific Vision gameplay and mechanics

Horrific Visions patch 11.1.5 take place in either Stormwind or Orgrimmar. Each of these two corrupted areas is divided into five zones:
- A central starting area
- Two corrupted side zones
- Two lost zones.
You’ll begin in the center and push outward, facing increasingly difficult enemies as they progress toward the final boss, Alleria or Thrall.
A full clear where you complete all five zones before defeating the final boss offers the best rewards, including high-value gear and exclusive cosmetics.
Sanity and life
A central mechanic is Sanity, a time-limited resource that drains constantly during a run. It’s reduced further by enemies, hazards, and Faceless Mask modifiers. If it hits zero, the run ends, regardless of health.
By default, you only have one life in Horrific Visions. However, with the right upgrades, you can gain a single self-resurrection, allowing you to revive once per run. You can restore or slow its loss with orbs and permanent talents that boost Sanity regen, capacity, and resistance.
Construct of Soridomi
Solo players or groups missing a role can summon the Construct of Soridormi to help out. Whether you need a Tank, DPS, or Healer, this companion can fill the gap, making Horrific Visions Revisited fully accessible, even for those who prefer going solo.
Progression comes through Displaced Corrupted Mementos, earned from enemies, chests, and objectives. These fuel a permanent Talent Tree offering upgrades like reduced Sanity drain, more damage, auto-revives, and utility buffs. You can also find temporary Tribute Buffs that grant short-term boosts during runs.
Faceless Masks and difficulty scaling
Faceless Masks are modifiers that you can activate before entering a Horrific Vision to increase the difficulty in exchange for significantly better rewards.
To activate these masks, interact with the corresponding altars before beginning a Horrific Vision. In group settings, only one player needs to have the mask equipped to activate it for the entire group.

The number of masks used directly impacts the quality of rewards. For example, completing a full clear with five Masks activated guarantees Hero-track gear, while eight-mask runs offer a chance at even rarer Hero-track loot. Certain achievements and weapon unlocks also require completing runs with specific mask thresholds.
There are eight masks in total, five of which return from Battle for Azeroth and three that are brand-new in 11.1.5. Each mask has a distinct effect as follows:
- Returning Masks:
- Faceless Mask of the Long Night: Reduces maximum Sanity by 50%, significantly limiting the time you can spend within the Vision.

Unlocking the old masks:
The four remaining classic masks, Burned Bridge, Daredevil, Pained, and Dark Imagination, are unlocked one at a time. After earning the first mask, you’ll need to complete a full clear with all currently unlocked masks active to unlock the next.
Here’s how it works:
- Complete a full clear with 1 mask to unlock the second.
- Then clear again with 2 masks to get the third.
- Repeat the process with 3 masks, and so on.
Keep progressing until all five masks are unlocked. Each successful, fully-masked run brings you closer to completing your collection.
Unlocking the new masks:
- Faceless Mask of Vengeance:
- Requires having at least one mask active.
- You need to find and loot it from a group of Vengeful Void Speakers located near the canals between the Trade District and Oldtown within the Stormwind Horrific Vision. You may need to interact with or defeat these NPCs.
- Faceless Mask of the Nemesis:
- Requires having at least one mask active.
- You need to defeat specific elite Nemesis enemies that spawn during the vision (Hogger in Stormwind, Gammon in Orgrimmar).
- In the Stormwind Vision: Look for a clickable object, specifically mentioned as a “Wanted Poster,” likely related to Hogger. Interacting with this poster should trigger Hogger to spawn nearby.
- In the Orgrimmar Vision: Look for a clickable Gammon axe.

Horrific Visions Revisited Corruptions: Rune Enchants

One of the features returning in Patch 11.1.5 Horrific Visions Revisited is Corruption, now reimagined as Helmet Rune Enchants.
Originally introduced in Battle for Azeroth, Corruption effects offered powerful bonuses but came with heavy drawbacks. In the 11.1.5 updated version, these Rune Enchants provide impactful bonuses and can significantly enhance your performance during runs. However, you won’t get any penalties.
