What are Horrific Visions

(Hourglass of Horrific Visions showcase. Source: MrGM)

In The War Within Patch 11.1.5, Revisited Horrific Visions are repeatable instances designed for solo players or small groups of up to five, with no specific roles required (there’s no strict Tank/Healer requirement). It brings back the popular “rogue-lite” scenarios from Battle for Azeroth, where you’ll enter twisted versions of Stormwind and Orgrimmar, corrupted by N’Zoth.

You’ll fight your way through five zones filled with enemies and objectives. Before your Sanity meter runs out, you need to complete as many objectives as possible and defeat the final boss:

or

Sanity drains over time and from enemy attacks. If it hits zero, the run ends. You’ll need to balance speed and strategy. The better you perform, the better the rewards, including gear, cosmetics, achievements, and more.

Horrific Visions 11.1.5 Release Date

Official The War Within patch 11.1.5 schedule announce

Horrific Visions are officially returning in World of Warcraft Patch 11.1.5, launching on May 20, 2025. This updated version revives one of Battle for Azeroth 8.3 activities. Although Patch 11.1.5 launches on April 22, Horrific Visions Revisited won’t be available until nearly a month later.

Horrific Vision gameplay and mechanics

Stormwind Guards in the Horrific Visions of N'zoth

Note

While you can bring your own group, keep in mind that Horrific Visions’ difficulty scales with the number of players.

Horrific Visions patch 11.1.5 take place in either Stormwind or Orgrimmar. Each of these two corrupted areas is divided into five zones:

You’ll begin in the center and push outward, facing increasingly difficult enemies as they progress toward the final boss, Alleria or Thrall.

A full clear where you complete all five zones before defeating the final boss offers the best rewards, including high-value gear and exclusive cosmetics.

Sanity and life

A central mechanic is Sanity, a time-limited resource that drains constantly during a run. It’s reduced further by enemies, hazards, and Faceless Mask modifiers. If it hits zero, the run ends, regardless of health.

By default, you only have one life in Horrific Visions. However, with the right upgrades, you can gain a single self-resurrection, allowing you to revive once per run. You can restore or slow its loss with orbs and permanent talents that boost Sanity regen, capacity, and resistance.

Construct of Soridomi

Solo players or groups missing a role can summon the Construct of Soridormi to help out. Whether you need a Tank, DPS, or Healer, this companion can fill the gap, making Horrific Visions Revisited fully accessible, even for those who prefer going solo.

Progression comes through Displaced Corrupted Mementos, earned from enemies, chests, and objectives. These fuel a permanent Talent Tree offering upgrades like reduced Sanity drain, more damage, auto-revives, and utility buffs. You can also find temporary Tribute Buffs that grant short-term boosts during runs.

Faceless Masks and difficulty scaling

Faceless Masks are modifiers that you can activate before entering a Horrific Vision to increase the difficulty in exchange for significantly better rewards.

To activate these masks, interact with the corresponding altars before beginning a Horrific Vision. In group settings, only one player needs to have the mask equipped to activate it for the entire group.

Note

The effects of these masks stack, meaning you can activate multiple masks at once to raise the stakes of a run dramatically.

The number of masks used directly impacts the quality of rewards. For example, completing a full clear with five Masks activated guarantees Hero-track gear, while eight-mask runs offer a chance at even rarer Hero-track loot. Certain achievements and weapon unlocks also require completing runs with specific mask thresholds.

There are eight masks in total, five of which return from Battle for Azeroth and three that are brand-new in 11.1.5. Each mask has a distinct effect as follows:

Buy Liberation of Undermine raid boost

Normal, Heroic, Mythic, single boss, mounts, achievements, up to ilvl 678 gear, and more! Self-play or piloted, your choice. 100% account safety guaranteed

How to unlock Faceless Masks in Horrific Visions

Now let’s get to the mask unlocking part!

Unlocking the first mask:

Unlocking the old masks:

The four remaining classic masks, Burned Bridge, Daredevil, Pained, and Dark Imagination, are unlocked one at a time. After earning the first mask, you’ll need to complete a full clear with all currently unlocked masks active to unlock the next.

Here’s how it works:

Keep progressing until all five masks are unlocked. Each successful, fully-masked run brings you closer to completing your collection.

Unlocking the new masks:

Horrific Visions Revisited Corruptions: Rune Enchants

N'zoth sitting is dark materia

One of the features returning in Patch 11.1.5 Horrific Visions Revisited is Corruption, now reimagined as Helmet Rune Enchants.

Originally introduced in Battle for Azeroth, Corruption effects offered powerful bonuses but came with heavy drawbacks. In the 11.1.5 updated version, these Rune Enchants provide impactful bonuses and can significantly enhance your performance during runs. However, you won’t get any penalties.

Note:

These new Rune Enchants are temporary and only available during Season 2 of The War Within.

How to unlock Rune Enchants

You’ll begin getting Rune Enchants by completing two questlines:

Once unlocked, Rune Enchants are purchasable from Augermu in Dornogal [35.0, 68.6] and are account-wide via your Warband. Lesser Runes cost 500 Mementos each; Greater Runes cost 1000 Mementos.

Horrific Visions: Lesser Rune Enchants

These Runes replicate classic Corruption powers in a toned-down form:

RuneEffect
Lesser Rune of the Void RitualYour abilities have a chance to increase all secondary stats by 109 per second for 20 seconds.
Lesser Rune of the Twisted AppendageAttacks may spawn a tentacle that Mind Flays your target for 36649 Shadow damage/sec over 10 seconds.
Lesser Rune of Gushing WoundDamaging spells may cause your target to bleed for 114660 damage over 7 seconds.
Lesser Rune of Infinite StarsSpells may trigger an Infinite Star for 100787 Arcane damage, increasing damage taken by 27487 per stack (up to 6).
Lesser Rune of Echoing VoidDamage builds Echoing Void, which collapses periodically, dealing 18324 Shadow damage/sec to nearby enemies.
Lesser Rune of Twilight DevastationAttacks may unleash a beam of Twilight Devastation, dealing up to 732998 Shadow damage in a frontal cone.

These are powerful upgrades that synergize well with various specs, offering a fun mix of visual flair and raw damage output.

Horrific Visions: Greater Rune Enchants

Greater Runes are upgraded versions of Lesser Runes, offering enhanced stats and stronger effects. They stand out as some of the most impactful gear enhancements available in Patch 11.1.5 Horrific Visions Revisited. These runes are ideal if you want to push higher difficulties, especially in runs with multiple Faceless Masks active.

RuneEffect
Greater Rune of the Void RitualBoosts all secondary stats by 151 per second for 20 seconds.
Greater Rune of the Twisted AppendageTentacles deal 50,727 Shadow damage/sec for 10 seconds.
Greater Rune of Gushing WoundBleeds the target for 158,697 damage over 7 seconds.
Greater Rune of Infinite StarsDeals 139,498 Arcane damage, increasing target’s damage taken by 38,044 per stack (up to 6).
Greater Rune of Echoing VoidCollapse deals 25,363 Shadow damage per second until stacks are gone.
Greater Rune of Twilight DevastationTriggers a frontal beam dealing up to 1 million Shadow damage.

Horrific Visions Revisited upgrade system

In WoW Horrific Visions Revisited, the upgrade system returns with a modern twist. Inspired by Battle for Azeroth’s Titanic Research, this version splits progression into two systems: the Hourglass of Horrific Visions and Tributes. Together, they provide long-term power gains for tackling Horrific Visions Revisited more efficiently.

Hourglass of Horrific Visions guide

The Hourglass acts as the primary upgrade tree, offering key survival and performance enhancements inside Revisited Horrific Visions. It’s a streamlined successor to the original Titanic Research system.

To unlock upgrades, you must exchange Displaced Corrupted Mementos for a new currency called Echoes of N’Zoth. This exchange is handled by Augermu in Dornogal. The system is timegated, allowing only a set number of Echoes each week—meaning it will take at least three weeks to fully unlock the tree.

Weekly Echo of N’Zoth Costs

WeekEcho costs (Cumulative Mementos)
120 / 100 / 300 / 500

Horrific Visions Tributes

Horrific Visions rewards

Horrific Visions in Patch 11.1.5 offer valuable weekly loot, scaling gear rewards, account-wide catch-up items, crests, and exclusive cosmetics.

One of the biggest rewards in Horrific Visions (Patch 11.1.5) is gear that scales up to the Hero track (ilvl 658)—but only if you can complete a full clear with all 8 Faceless Masks active.

Each week, you can earn one gear reward per unique mask difficulty tier, per character. The more masks you use during a full clear (all five zones + final boss), the higher the item level of the loot:

Masks ActiveReward TrackApprox. Item LevelGear Type
0 MasksVeteran~626–636LFR-equivalent gear
1–2 MasksChampion~639–645Normal-tier gear
3–4 MasksHero~648–652Heroic-tier gear
5 MasksHero~658Heroic BiS-level
6–8 MasksNo higher ilvl(Same as 5-mask)Prestige/challenge only

Tip:

You can do multiple runs per week, but only the first run at each difficulty (mask count) grants gear. Plan your clears to hit 4 and 5-mask thresholds first for optimal loot.

Each Horrific Visions run grants Undermine Crests, which are used to upgrade gear through the Season 4 upgrade system. These crests help boost your overall item level, making Horrific Visions a strong option for both catch-up gearing and endgame progression.

The type of crest you earn depends on the number of Faceless Masks you have active during your run:

Masks ActiveCrest Type
0 MasksWeathered Undermine Crest
1–2 MasksCarved Undermine Crest
3–4 MasksRuned Undermine Crest
5+ MasksGilded Undermine Crest
